Centre Dentaire Ville Mont-Royal Inc., operating under the commercial name Centre Dentaire Oralvie (“Oralvie”, “we”, “our”), is committed to protecting the personal information you entrust to us. This policy describes the information we collect, why we collect it, how it is used and what your rights are. It complies with Quebec’s Act respecting the protection of personal information in the private sector, as amended by Law 25.

Information we collect
We collect only the information necessary to provide our dental services and to manage our relationship with you:
- Identification: first name, last name, date of birth, gender, address, email, phone number.
- Health information: medical and dental history, medications, allergies, x-rays, clinical photos, treatment plans, clinical notes.
- Insurance information: insurer name, policy number, certificate number, coverage.
- Payment information: payment method used, transaction history. Credit card numbers are processed by our payment provider and are not stored by Oralvie.
- Browsing information: when you visit our website, certain technical information (IP address, browser type, pages visited) may be collected via cookies. See our Cookie Policy.
Purposes of the collection
Your personal information is used for the following purposes:
- Providing the dental care and services requested.
- Communicating with you about your appointments, treatments and follow-ups.
- Submitting claims to your insurer, with your authorization.
- Managing billing and payments.
- Answering your questions and requests.
- Meeting our legal, regulatory and professional obligations (including towards the Ordre des dentistes du Québec and RAMQ).
- Improving the quality of our services and the security of our website.
Consent
We collect, use and disclose your personal information only with your consent, except where the law authorizes or requires us to act otherwise. Your consent is obtained at the time of collection, whether in clinic, by phone, by email or through a form on our website. You may withdraw your consent at any time, subject to legal restrictions.
Disclosure to third parties
We never sell your personal information. We may share it with certain third parties strictly in connection with the delivery of our services:
- Dental laboratories and specialists involved in your treatment.
- Insurance companies, with your authorization, for claims processing.
- Clinical and administrative software providers (electronic patient records, appointment system, patient communication), acting as processors and contractually required to protect your information.
- Public authorities or professional orders, when required by law.
Retention and destruction
In accordance with the requirements of the Ordre des dentistes du Québec, your clinical records are kept for a minimum of five years after your last visit, or longer if required by law. Non-clinical information is kept only for as long as necessary to fulfill the purposes for which it was collected, then securely destroyed.
Security
We implement reasonable physical, technical and administrative measures to protect your information against loss, theft, unauthorized access, disclosure, copying, use or modification. These measures include restricted access to records, encryption of sensitive data, and staff training.
Your rights
Under Law 25 and other applicable laws, you have the right to:
- Access the personal information we hold about you.
- Request the correction of inaccurate, incomplete or ambiguous information.
- Withdraw your consent to the collection, use or disclosure of your information, subject to legal restrictions.
- Request the de-indexing or the cessation of the dissemination of certain information about you, where provided for by law.
- Obtain the portability of your information in a structured, commonly used technological format.
- File a complaint with the person in charge named in section 2, or with the Commission d’accès à l’information du Québec (www.cai.gouv.qc.ca).

Privacy incident
In the event of an incident involving a risk of serious harm, we will take the required measures to limit the consequences and will notify the Commission d’accès à l’information du Québec and the affected persons, in accordance with the law.

We may update this policy from time to time. The date of the last update is shown at the top of this document. We encourage you to review it regularly.
